District Simplified Grants


District Simplified Grants for individual Rotary Clubs in District 5190 can be found only on this web site. The Rotary Foundation Manual "The Guide to Humanitarian Grants" 144-EN describes the other various grants offered and the application procedures for each. Updated information is available on the RI Website using links above.

The required grant forms are found at the links to the left. Please note that, if you have Microsoft Word, then choose the "DOC" format so that you can type directly onto the form on your computer. "PDF" forms can be printed and then filled in by hand. Either can then be signed and sent by fax.

All Grant applications must be submitted to the District Grants Subcommittee to be reviewed for completeness. The contact information for the District Grants Subcommittee members is HERE. Find your club and select the email link for your District Grants Sub Committee member. NOTE: you will have to login as a Guest to see the email links.

Once the District Grants Subcommittee certifies the application complete, the application will be forwarded to the appropriate recipient. DSG's will go to the D5190 Foundation Chair; All other Grants will go to the Rotary Foundation.


District Simplified Grant Reports:

Progress reports are required every six months. A final report is required upon completing the project report form. The same DSG Report Form is used for both the progress reports and the final report. Those forms are available from the menu on the left side of this web page.
